New Client Intake

Step 1: Gather all required documents needed to complete the New Client Intake Form. This includes:  

  • A prescription from your physician 
    • The prescription should be from your Primary Care Physician and should contain your child’s diagnosis and the type of therapy you are looking to begin 
  • A copy of the front and back of your insurance card 
  • A copy of the front of your driver’s license 
  • For ABA therapy only, a copy of your Diagnostic Paperwork.  
    • This should be from a psychologist, psychiatrist or neurologist. This usually includes specific testing results and an identified diagnosis at the end of the report. 

*You will not be able to complete the New Client Intake Form without uploading these documents. Please make sure you have them available in an electronic format prior to starting the New Client Intake Form in Step 2. 

Step 2: Complete the New Client Form 

STEP 3: Our Front Desk Specialists will review your submission and will reach out to you via phone or email within 48 hours to confirm that your submission has been accepted or to obtain clarification or additional required information. As a courtesy, we will verify insurance benefits. We ask that you also call the 800 number on the back of your card to clarify your coverage and understand your financial responsibility (copay/coinsurance/deductible). 

STEP 4: Once our scheduling team identifies appointment time(s) that match your availability, they will reach out to you to schedule the initial evaluation. You will receive an email confirming your appointment for the initial evaluation which will also contain a link to the Evaluation Intake Form. You should complete this form prior to your scheduled appointment.

  • How long do I have to wait for my child to be scheduled? 
    • There are many factors that influence our ability to schedule new clients. It is difficult to provide an accurate estimation of when we can schedule your child’s evaluation and ongoing services. Please understand that we are consistently working to schedule clients as soon as possible. Wait times may vary from a few days to a few months.  We will work to keep you informed of availability.
